Edit: Nevermind, the glitch disappeared after removing game cache and repairing the game via origin. :)
Did that add the missing ghost back into your tree?

Yes, it did. I think just the thumbnail picture was broken. When I moved the cursor over the broken picture, it still showed the name of the ghost.
