When a child is selecting their grandparents to chat an option "adopt as a caregiver" appears. Which normally only appears for YA+ on adopted children. And it appears only on grandparents, for parents everything is fine and as usual.
I don't think it is a bug but a game mechanic. It enables grandparents (or other "non parents" such as older siblings) to care for the toddlers and children better. I have used it a lot with my sims. I often move toddlers and children to the grandparents household to spend a couple of days with their grandparents. Then the parents also get some time for themselves and can go on a vacation together or something. Anyway, I don't remember when I first saw it (maybe a couple of years ago), but at some point I noticed that especially the toddlers wouldn't cooperate with grandparents unless the grandparent picked the option "adopt as care dependent" (sometimes it was already picked for them) and the toddler chose the "adopt as caregiver" option. I have had teens take care of their younger siblings like this too.
