Was the proposal outside per chance? For some reason, others report the proposal will succeed if it happens indoors, but fail outdoors. It's definitely a bug. I've had the same thing happen to me in the past and just a couple hours ago at the Romance Festival. I forgot about the bug. D'oh! Felt soo bad for my Sim, she was very embarrassed. o.O But, I realized what happened this time and they're back together.
