It definitely would not raise the rating above T (as long as they didn't make the animation especially gory, which they obviously wouldn't), so I don't see why this was not in the pack. I feel like it's another example of The Sims 4 being made into a way too happy-go-lucky game where nothing ever really goes wrong. And I'm getting so tired of it.
