I just had something similar happen the other night. My sim only had a +2 playful buff and suddenly he turned hysterical and died. Then I had an issue with grim not appearing. The sim who died from laughter had like 20 death flowers in his inventory so I wasn't too concerned it. I gave his roomate the death flower to hand to grim when he arrived but he never did. Grim never appeared and I just got an urn instead.
So I exited and reloaded. I had saved just before he turned hysterical and it happened all over again. It was Love Day and all he had was +2 Playful from something. I use MCCC, so I was quick thinking and flagged him as immortal so he wouldn't die again, which worked. I think something is definitely off with emotions going haywire. My situation sounds similar to your experience with the hysterical death.
