I have two sims in love but rarely ever do romantic interactions. One is a romantic and he gets flirty often but he always did because of his trait. The other is childish and paranoid and she's almost never flirty unless I'm purposely doing something.
And they both have close sentiments with eachother. They more so get happy from being in eachothers company but not flirty.
The icon is two roses and a book with the title 'utterly enamoured' both sims get the same icon when they are in the same room, the book in the icon symbolises the Sentiment feature. When I hover over their Sentiment in the conversation bar it says 'deeply in love, enamoured sentiment'. They don't have romantic or alluring traits, I'm pretty sure it's the Sentiment that's causing this and it didn't happen until this feature was added.
