My sims seem to get the unpleasant surroundings moodlet the instant they go into the vet clinic. Yet as far as I can tell, it's clean each time.. I've also noticed that whenever they open the clinic and let their employee work alone they have up to 7 patients at the end of the day. But if they go in themselves, they only have two. Of course I keep interrupting them so they'd work instead of standing around chatting.
