I think the image is real, but lilsimsie didn't leak it. If she did, EA allowed her to, but honestly, I don't think she leaked it at all. I mean, somebody on these forums would have seen her stream, right? I think somebody just photoshoped her in.
But the world does seem accurate, and the area with the bridge matches the render.
Two things I've noticed, though, is the fact that there's a karaoke lot, and karaokes came with City Living and also I couldn't find on the map the area where the mountain climbing shown on the render could take place.
