I'd love wheelchairs, I wouldn't hold my breath though considering the animation budget it'd no doubt take.
One of the things I really like about the insane trait, is that, while it obviously plays mostly for laughs my sim will often say mean or inappropriate things for no apparent reason. Obviously tedious to deal with but that capacity to let you play with someone different from the norm and have it reflected in actual gameplay is something I absolutely cherish.
