Last time I saw someone with this issue, they needed to redownload the affected CC.
Oh Lord I hope that that ain't it, I have 91 GB of cc lol. Plus I've been downloading new stuff from Curseforge recently and they're blank thumbnails in game too.
Have you tried renaming the folder, deleting the localthumbcache file, and then downloading a few new items? Consider also that that MASSIVE volume of CC is extremely likely to have duplicates and broken content, plus to have issues just from quantity alone. I really recommend a fresh start and limiting your CC to what you will be actively using in-game.
