Try moving your mods folder to the desktop. Then start the game and a new mods folder will be generated in the sims 4 folder. Exit the game. Then put your cc back from the desktop mods folder into the new mods folder in the Sims 4 folder , and see if the cc appears then.
If the issue remains, the cc may have been broken through a game patch. Try downloading and installing a different cc item and see if that cc appears.
