Not sure this is a bug. The game has added phantom parents before but since yours happens at a specific time, I need you to test on a vanilla game.
To verify on a vanilla game, move your mods folder to the desktop and delete the following files from your user folder.
- localthumbcache
- Any lastException Files
- Any LastCrash files.Test a backup of your save as you will loose all cc on the save and see if the same error occurs, then report back.

Testing it in a fresh file with no mods or cc. My sims are expecting, but it'll take a bit. Will report back when I look at the tree after the baby is born.

Hmm.... The only big mod I have is MC Command Center, but not in my test file. Must be something in that mod causing the invisible grandparents

@crininct can I send you my settings for MCCC? I tried to get their help, but they weren't listening.

I'm going to move this to mods. I don't use MCCC myself so I can't help.
Please check that you're on the most recent version of MC Command Center - 2025.3.0 - and that you have no other MCCC versions also installed. Once you've checked that, please test what happens if you have only MCCC installed, no other mods.

UPDATE: It only seems to happen if you begin a family with a single sim and move that sim's partner into your active household. I remade my sims as a couple through CAS and they have two sons..... No added "grandparents". A friend of mine told me she had a similar issue when her main sim married a non-active sim and he moved into her active household.
