If you follow the link to coolspear1's mod that I posted above then he lists which insects are spawned with each spawner.
Also, yes, the insects require terrain to appear so that is why you would have been unable to spawn them on a second storey. Also, some insects only come out during the day. After 6pm they stop spawning until the following day.
