Limbs Randomly Disappear

I have been recently suffering from a problem where sims limbs randomly disappear. Sometimes it is a hand, foot, arm, or a leg. They just look like it has been cut off.  It happens to sims both my played and unplayed household. It happens in live mode and in create a sim mode, too.

I have done fifty fifty, repaired my game, played vanilla and the problem still persist. Some of my friends have the same issue too. 

I do not have any body presets, body mods or sliders. It happens on sims that have cc on and no cc and happens when the game is vanilla too. 

Temporary fixes I have discovered include restarting the save or game, entering create a sim mode and resetting the body to the default and editing afterwards or changing their outfit. 

I have not seen any fixes other than what I have mentioned and I think because this issue happens sporadically it is very random no pattern to it it is hard to pin down the cause.
