I have a hard time playing without the No Fatness / No Muscles mod: http://modthesims.info/d/534028 (it disables them changing through gameplay)
I work hard to get my sims looking how I want them to in CAS, with very specific clothing choices to match their body type and all that jazz. So if their body type changes too much, it can ruin the look.
Plus, there doesn't seem to be actual base body types beyond male/female. So if you get super muscular, it doesn't matter where you started, you'll look the same as someone who was given super muscles in CAS. Part of the reason I play with it off; in RL, through weight and muscle change, people still (mostly) stick to the same base body type and that doesn't seem to be something this game does.
