So that the Mods/CC possibility is eliminated more thoroughly. Please create a new game save, save it, and leave the game. Then try the following, but only open the game in the new game save to test, and don't save while or after testing this. That way you can check for CC/Mod issues in a more separate area. Do all steps, even if you did some before:

After you created that new game save and have exited the game...
Move your Mods folder from your "The Sims 4" folder [Documents > Electronic Arts > The Sims 4] to your desktop (save it for reference) and delete the localthumbcache.package file in your “The Sims 4” folder. Then check in the new game save to see if this issue is still occurring. If the issue is gone, please re-add the Mods/CC one at a time until you find the one causing the issue. If you have a lot of Mods/CC you can try the 50/50 Method. You can test without saving in order to identify which CC/Mods are causing the issue, then remove or add updated version(s) of the Mod/CC causing the issue.
