@Olin_Ozin Highlight the Sims 4 on the home screen on your PS5, press the options button, then select "manage game content" and the PS5 will bring up a list of all content for that game that you own.

I’ve tried that and it says all content installed but when I go into the game it doesn’t show any of my expansions.

Do you have multiple account on your PS5? If yes, you need to enable console sharing on all account. (Settings, Users and Accounts, Other, Console sharing and offline play.)
