BobTheBuilder015 Are you using the cas.fulleditmode cheat by any chance when this happens because some cheats are causing screen and menu freezes that require you to exit to be able to play.
If not, when this happens have you tried powercycling your Xbox, Switch it off with the Xbox button on the front for around 10 secs until it's shut down completely and the light stops flashing. Unplug the power cable to completely reset the power supply. Wait at least 10 secs then plug it back in. Switch the Xbox on with the power button. I'd also try reinstalling your game and content and see if that continues.
