Start with a clean TS4 folder - [INSTRUCTIONS] How to troubleshoot and test a broken game | EA Forums - 11864548
Play in a brand new save and see if you get the same issue. If not, copy your save from your old, renamed Sims 4 > saves folder into the new Sims 4 > saves folder and then re-launch the game and see if it happens again with that save.
