Which version of macOS are you running?
Have you repaired the game?
Have you run the Origin reset tool? https://help.ea.com/en-gb/help/origin/origin/how-to-reset-origin-on-mac/
Have you moved the Sims 4 folder from Documents > Electronic Arts to the desktop and tried to launch a new, clean game?

Hi!
I'm running Catalina 10.15.7. I just reset Origin and now can't find my external hard drive. I repaired the game files multiple times. I no longer have an EA folder. I lost it a while ago.

Okay, so I found an EA folder in my files, and moved it to desktop, but the game is still saying it's already running. I can't find my external hard drive through Origin either.

Moving the folder only works if you move the Sims 4 folder which is in the Electronic Arts folder in your Documents folder as that is the file path the game follows to load your user data such as saves.
Repairing the game wouldn't affect the location of the external drive. It sounds like the install has gone awry. Here's my step by step guide on getting Origin to see your external drive http://bluebellflora.com/2020/11/26/external-drive-not-showing-in-origin/
