Disabling or turning off both mod/cc options isn't enough of a test.
You need to move the mods folder to the desktop and clear the game cache and delete the localthumbcache.package from the Sims 4 folder to help with troubleshooting.

Testing like this will help determine if it's a mod issue, cc issue or a vanilla game issue.

It is not an issue with the mods/CC.
Did you test this on a fresh save as well ? Is it happening there as well ?
Yes, I did. I deleted my old save and tried again. It's still happening.
Have you tried resetting your entire user folder yet ? There have been a few suggestions made in that direction so I'm not sure if you tried that.
Just rename the user folder, then try on a fresh save.
