Its still happening. I enabled cheats and attempted to add him but there was no option for him. I could add other sims to the household but not him. Also her relationship with him was gone, like they had just met. I dressed him as well and he had hair in CAS but they disappear once the game has loaded after I move them onto a lot.
@michelle_renee86 I tested your save and couldn't duplicate any oddity but I just helped a user in the German forum, that had the same weird UI glitch you have on your picture.
In that save it helped to load another household first, then load the faulty one.
They only had the issue on travelling though.

I tried to load another household before theirs and I reuploaded the family to the gallery from my Xbox and redownloaded them to my PC game, just to be sure, but nether approach worked. I also tried to have one of my other sims marry him(she did) and then I locked all doors specifically for him, but when I went to manage worlds or CAS, he would simply disappear, though the doors would sill be locked, she'd still pregnant and have the just married moodlet, he was not in her known relationships or households not in world section of manage worlds. So I figure my game was just determined to delete him for some reason. So I ended up going to create a sim adding the family(minus the pets) and one by one in creating a twin for each sim and deleting the original and giving them the original sims name, traits, and aspirations. I then placed the on a lot and built them a house and played them for an additional 45 mins. It seemed to work fine, I had full control of all sims in in the household and none of them had the broken UI. If there is another potential solution I'd be willing to try it otherwise I guess this will have to do for now...
