Once you've uninstalled Sims 4, remove the EA App as well, as described. Restart your computer, reinstall the EA App, and install Sims 4 to the default location:
C:\Program Files\EA Games\The Sims 4
The other locations are a problem for various reasons, and it's also important to get rid of any hidden files that point to these locations, which is why you need to use Revo rather than simply uninstalling through the EA App.
For the part about moving Documents, you can try the easy way first, if you want. It doesn't always work, in fact I've seen lots of reports of it not working, BUT using it won't break anything. Open a File Explorer window, and in the left panel, you'll see a Documents folder with a special icon (not the normal yellow one). Right-click that icon and select Properties, then the Location tab. Click Move.
Next, click This PC, then Users > your username > Documents. Don't open this Documents folder; just select it by clicking once. Then click Select folder at the bottom of the window. This guide has screenshots:
