I saw same kind of clothing/hair icons in The Sim Supply channel video here and he said his "more columns in CAS" mod wasn't up to date. I know it's not all what you are experiencing and you say you had this problem with a new game, where you had none of the old files. RIght? So there is no old cache files either? I'm hoping maybe this information might help someone more technically advanced person to get some clues what this might be about.
I'm sorry for asking stupid questions, but I didn't fully understand if you have a new The Sims 4 folder now in your documents folder or you just replaced some files inside of it? When I encounter problems with game, I rename my The Sims 4 folder found in C:\Users\<user name>\Documents\Electronic Arts\The Sims 4\ as "The Sims 4_backup" or "The Sims 4_glitch" or something like that so I know what folder it is, and when I start a game it makes a whole new The Sims 4 folder that has absolutely no trace of old game files, no cc, no cache, nothing. Then I can test if the problem was some corrupted old game files or cc, or not.

After each update, most scripts mods and a large part CC will be spoiled and will spoil the game. Checking what broke down this time after the patch, and replacing these broken ones by a new updated version or deleting these is the price the player pays for using CC and mods
Move the mods folder to the desktop.
Delete localthumbcache.package file in the game - important!
Fix the game in Origin.
Origin/Library - right-click on TS4 - option "Repair game"
You have to wait until Origin will fix all packs, not only the base game.
Turn on the game to create a new, empty mods folder and check if a game is working now without mods and CC.
Check if there are new updated versions of the mods and CC you are using.
Then copy from desktop your mods and CC and paste it in the new mods folder. First, paste script mods one by one every time checking if the game is OK. Do the same with CC but if you have a lot of this, try the 50/50 method. ( divide your CC into smaller batches for example 50 pieces in one subfolder and check these subfolders one by one)
You need to find a culprit and either remove it or replace it with an updated versionI don't work for EA

Thanks for the help! Unfortunately, this didn't work, but I tried this before when I was having trouble with something else and it worked. I think I'm just going to have to play an older version of the game.

In most cases it's not the MODS, it happens almost every useless patch that they put out. The patches has given gamers more issues that they're worth

I get what you're saying but even though I have the "more columns in CAS" mod, I tried my new game without CAS and it still was glitchy. I have the new Sims 4 folder but I basically kept all of my save files and mods. before I put them back in the game I tested it without any of my previous stuff and it still was broken :/

Reinstalling the game will not fix the files in the old game folder because it is done in the installation folder in Program Files, and your current game is in the second folder in Documents. To repair files in this second folder you have to do a factory reset as described in this post:
