The only thing I see is an Intel graphics driver, are you sure you have an GeForce?
Your Intel drivers are from 2012, I would suggest updating those.

@gmk101 Do you have the GTX 650 installed? It is not showing up in the dxdiag. The Intel GMA 3100 with 0Mb of dedicated memory is not supported. Do you have your vido cable plugged into the GTX 650? You have Windows 10 32 bit so your computer can only access 4Gb of memory and yours have 3Gb. While installing 4Gb of memory will not supply a complete 4Gb because rom images overlay the upper limit but even that little bit more can make a big difference.
add: did you upgrade the 300W power supply? Nvidia recommends a minimum 400W (I would recommend at least 500W). Your computer uses DDR2 memory modules. DDR2 is dead. DDR4 is now out which means it is a matter of time when DDR3 will start to fade.
