Thanks for the dxdiag, which lists a number of crashes of the graphics driver. So please do a clean uninstall and reinstall of the driver, which is more thorough than simply updating. Here's how:
If this doesn't help, the issue could be hardware-related, especially given the age of your system. But as a next step, I'd suggest installing a program like GPU-Z or hwinfo that can monitor the temperatures of your graphics card. GPU-Z is lightweight and mostly looks at GPU data; hwinfo captures all the sensors it can find. In either case, let me know what kind of temps you see when running the game. The max should still be listed even if you get a crash before you can check with the game open.
