Sims not leaving Apartment

Product: The Sims 4
Platform:PC
Which language are you playing the game in? English
How often does the bug occur? Often (50% - 99%)
What is your current game version number? 1.61.15.1020
What expansions, game packs, and stuff packs do you have installed? Get To Work, Get Together, City Living, Island Living, Seasons, Cats and Dogs, Vampires, Dine Out, Spa Day, Outdoor Retreat, Laundry Day
Steps: How can we find the bug ourselves? Try to let a sim "go downstairs/upstairs" (/use an elevator) from an apartment
What happens when the bug occurs? The sim doesn't go downstairs/upstairs when told but protests by saying that there's something blocking their way
What do you expect to see? The sim should move down-/upstairs without any problems
Have you installed any customization with the game, e.g. Custom Content or Mods? Yes
Did this issue appear after a specific patch or change you made to your system? Neutral/Not Sure

I'm not sure if this issue always occurs but in most cases. 

i tried many things, but nothing really helped except moving all my mods out of the game

As i don't really like playing without them, i'm looking for an easier solution to this problem.

Another thing, whether it is somehow connected with the going up-/downstairs bug is kind of a hole in the map of San Myshuno (see the screenshot). this also occurs in the most basic status of the game:

    I have had this problem before and it was caused by wall placement at the top of the stairs. I copied the upstairs. Deleted the upstairs and then the stairs themselves. Put the upstairs back. Checked the walls, roofs, etc and then put the stairs back. This resolved it. My roof of the upstairs was blocking the top of the stairs but I couldn't see it until I removed the upstairs.
