I also have the same issue I tried many times and I still get this error.

Connection type: cable 

ISP: O2

When does it start happening? (e.g., during matchmaking, mid-match, post-match, between rounds) : it was post match each time

Squad changes between rounds: Last time it was 0 or 1

Are you switching squads via the squad selector after each round (actively choosing a different squad): not always 

Are you staying on the same squad and swapping players manually / using Squad Builder? Yes

Anything unusual right before it started? (any detail that could help reproduce the issue)
