Head menu broken

I play on PC and bought the ultimate edition through Steam. When I try to create an avatar in either a Career mode or in Clubs when I press RB to get to the "Head" menu I can't select anything with my controller. I can still use LB,RB to go back to Info, and RS to rotate the player but not LS or D-pad to select a head. If I then swap to mouse and keyboard I can select a head and press enter to edit the head at which point it constantly cycles through the sub headers (Skin, Head, Face, Hair) making it impossible to select to change anything (the controller still doesn't work on this screen).

I don't know if this is related but the controller I use comes up as the second or sometimes third controller connected when I play despite it being the only controller connected to my PC. I have occasionally had the issue in manager career where playing youth tournaments it will auto select the "first" controller which means I can't play because that controller doesn't exist.
