Happening in my save too while playing without mods. Any sim I have used the paint tool on is completely black now and there are a lot! I've played extensively with this feature, and when it was introduced for sims in the Businesses and Hobbies I went to town experimenting on my sims faces and skin. Also noticed in a YouTubers early access video that customised fairy wings also turned black.
