Constant death notifications for household sim (former townie)

I have a household sim that was a townie. Every couple of days he gets a death notification call that his sister has died. She's very much alive in the game, he can call her and visit her and she does not appear as a ghost in his relationships tab. She has "died" about 8 or 9 times and every time he and his wife mourn her. Visiting or calling doesn't remove the "very sad" emote.
