I managed to capture a save using only the base game and EBN with no mods and a freshly repaired game. The entire user folder was relocated to generate new files prior to starting.
I've been tracking this issue for the last month and have found a few things to note. These debug items can be made visible with a couple ways in Build/Buy mode.
If it's outside, you can select "Move Lot (Y)" and their bounding box will be shown along with everything else. They appear as small green dots that appear to be attached to nothing when canceling the operation. Note harvestable plants also generate separate items of similar size. Debug items inside a room are made visible entirely when you select and attempt to move a room. While a preview of the room with all items inside it are on the cursor, you can still zoom in on the preview and use the camera controls to inspect the area. On PC, if you hold the right mouse button by default, it'll hold the room preview to the grid and you can use WASD to move the camera around, moving the mouse will still spin the camera around (mouselook) as normal.
Here's a sample save file where these items spawned. There's only one played household, just load that and look at their home lot. There's a couple of the debug items located on the walkway in front of the front door. This is considered a room, so you can spot them by attempting to move it around.
In my time of testing. I only had observed these showing up in areas that Fairy Dust had been generated on lot, both by a fairy bathing themselves and fairy woohoo. It seems intermittent at best with a relatively low chance of happening per interaction. It took almost 4 hours of gameplay to capture this instance.
Users encountering these can get rid of them by building a room around them and deleting the room. They do get moved to household inventory if you manage to build a wall over one, but this isn't reliable as they can spawn in spots of a tile where a wall can't intersect.
