This happened in a custom venue with ghosts in human form. They still have their abilities and glow while using them, but look like normal living Sims.
Game with latest update, no mods, playing on pc.

Is that a role you made ? Is this only an issue on the custom venue or do they appear the same way outside of them ?
I did not test this outside the custom venue and it is one of the roles for the custom venue. I have ghosts in this kind of cemetry/crematorium to spook a little the visitors. This is the first time reionizing this, by my search about it I found other people talking about ghost problems as example in the reaper career are ghosts for someone as humans too.
Yeah there's several causes which I'm wondering if this particular one is showing as ghost outside of custom venues.
There does seem to be a main issue where Sims outside of the household don't become proper ghosts but it can also have to do with missing death types, especially for non-adult Sims.
Can you switch to the household and see what he supposingly died of ? Either try on a backup or don't save your game.
I could opend the "ghost" Sim in CAS and there he is not a ghost at all, after canceling out I was treeted with a infinity loading screen.
My guess is for a custom venue, if there are not enough ghosts with the right criteria, the game just lables Npcs as ghosts and let them act as such. But this is just a guess.
More I did not check, but when needed I can send you the save file for your paid gametesters.
I'm not a paid tester as I don't work for EA but it would be interesting to take a look at your save.

Willow Creek is the only neighborhood that has lots right ? Didn't check all the worlds but I can't find tower of death. did you mean tower of the lost ?
Also is that teen only from your own game ? I assume you downloaded that save and then started playing with it so it's not exactly the same as yours ?
OK. Did some testing on that save and on a new one and seems there's something really bad going on here.
It's seems it's all occults and not just ghosts.
What I did is create a custom venue that had 4 Vamps/4 Werewolves/4 Ghosts and 4 Aliens. Then I deleted all the werewolves from the save and made sure each of the other categories had < 4 already in the game.
What happened was that the game created new Sims that have the trait of the occult while being human at the same time.
So yeah, def a bug. If the game doesn't find enough occults, it creates half-occults that are then .. broken.
