The infant milestone 'First visit to a family member's home' does not unlock when visiting a family member's home. When you reload the lot while you are still there, for example by exiting and reopening the game, the milestone does unlock.

Same happened to me. I thought it could be because the family in question is an alien baby. Reloading the game worked.
Community ManagerInfants will now properly unlock the "First Visit to a Family Member's Home" Milestone.
