i had this same issue. weeks of having them play in the toy mat, my youngest infant would get the shine animation, sound, etc and sims would come 'watch the milestone' but it never registered in their simology.
i selected the child to play as them, did testingcheats true, shift+click child->remove from family, shift+click->add to family, save, exit to desktop, reload and it fixed it and started getting milestone progression again

Can confirm this works, just had a similar issue with milestones playing animation and sound but not loading in or registering as milestones in the menu. Did the fix above and immediately got lift head milestone after doing tummy time.

Yay! Removing her from the family and putting her back in worked. It is giving her milestones now, but is not registering the ones she has already done. But thank you. You helped!
