this happened with my new infant. I made a separate save right before she was born and went back to that save, even if that meant I lost progress on my current file, because I had seen so many milestones pass by without them registering and felt sad because I wanted to see her childhood progress.
main point: everything was going well after her birth, new milestones were registering, but after the father went off the lot to his reaper career, he came home to the mother witnessing a milestone but with no actual happening/notification all over again.
I doubt everybody who has this problem has the reaper career or even has sims who go off lot for careers, but it could be part of the problem and if the devs can use this info, I hope it helps.
I've had it both with parents in the reaper career and without, and most of the time my YA and adult sims don't have "jobs" they collect and/or create stuff for money. My parent sims generally don't leave the lot until the kids are school age and go to school. Mostly because I didn't know you could until recently lol
But any information is good information to have!
