Ideas
When playing with Fairy infants and toddlers, only a couple of milestones (such as being adopted, toddlers getting their first nightmare) are earned. Infants otherwise will get the "achieving milestone" but won't sparkle and the notification won't show up. I haven't play-tested children enough to know if they're actually affected as well, but I'm including them just in case since younger Fairies seem to be bugged in general.
For infants, I noticed things like having first bath or bubble bath, first blowout, rolling onto stomach/back, etc. aren't triggering the actual milestone being earned.
For toddlers, throwing first tantrum, shapes, learn to talk, maxing potty, etc. aren't actually being earned.
I do play with with CC and mods, but I make sure to keep them updated to avoid interference. Using weerbesu's UI Cheats Extension is the only way they are being unlocked.
All packs are enabled and I've seen Get Together build/buy/CAS content within the game.
The infant-child has plenty of space to achieve the milestone so it shouldn't be a spacing issue.

I didn't realize that there was already a similar bug report that's been around since Growing Together first came out. So, for some further information:
I wasn't having any issues with milestones before Enchanted by Nature came out. They were even working with fairy infants and toddlers until recently, so I'm guessing one of the latest two patches broke it for me personally. I haven't tried testing milestones on any other occult or regular Sim infant and toddler since EBN came out since I've mostly been playing with fairies.
