It always triggers right as they get dumped back to earth. No fear before abduction, they arrive home and immediately, fear kicks in. Notification makes it hard to miss the connection.
If you mean, had they previously had the fear on any other occasion, I can say with certainty that one of them has never had it for non alien abduction reasons, and I'm 95-99% sure that's the case for the other three too.

yeah, that's what I meant as the fear of dark is bugged and comes back even if you cure it so wanted to make sure that's not your issue.
Okay, yeah, I think in this particular case it's an abduction specific thing, as I've been using the satellite to prevent abductions for a couple of in game weeks now (6 alien infants at once on long lifespan is MORE than enough thank you very much) and I don't believe anyone has had that fear in that time.
