This happened to me too. My child sim was going to school, when I got a pop up saying he passed away. Then I got a notification saying he was being taken away. No urn, not in the family tree, he was just deleted. Just like OP, my child sim had a good relationship with his parents, A student and full needs. I don't know if this is important, but he also had neutral confidence.

Removing mods doesn't seem to work.
