This happened to me just now. It's definitely related to LD because I recently acquired the pack and played for the first time with this save (a fairly recent save). Before that, everything was running smoothly, my Sims were already in the child life stage before having the pack.
I don't have any mods related to death installed.
Both children (twins) are grade A students, needs are always in the green, have finished one aspiration and have neutral confidence, top-notch infant, top-notch toddler and high character values.
Only received the notification of one of them feeling neglected (which I found weird), but afterwards, both were taken away / died?? one after the other. Their energy need was yellow when it happened (if it can be relevant).
Can't really play the game with this happening... Hope this is fixed soon :/
