I'm also having this issue! I noticed that he changes to the appearance I customised right after I leave CAS, but if I travel to a different lot he resets to how it was when I got him from Cupid's Corner with a female voice. He's currently an elder in my game but the appearance keeps appearing exactly as he was from Cupid's Corner, a young adult, even though he has elder animations like stretching the back and all.
