I recently had the same thing happen with Gibbi Point. the one thing I did notice is that it took place while my game was trying to download new content. I don't know if that helps. All the games I play that were not created while content was downloading or had just freshly downloaded didn't react this way. Specifically the Adventure Awaits pack had just loaded. I wonder if it's not fully loading and then trying again, thereby creating an extra copy. If others have had it happen at a fresh content download maybe chime in.
