I've had it on two separate saves now where my married lesbian couples, both of whom had children, suddenly gained the siblings relationship. This happened at a random point when I loaded up the game, and I was unable to remove this relationship with either MCCC or UI Cheats, both of which I only installed after everything else failed in an attempt to resolve the issue. I have no idea what causes this issue to happen.
And this issue wasn't fixed in the most recent 150+ bug fix patch. 🙄
