I'm having the same issue. My two married male sims were romancing just fine, then I went into CAS after their daughter aged up. After exiting CAS my spouses were not longer able to interact romantically except to give/receive massage. I also noticed that the were now labeled as brothers.
I downloaded MCC and removed the brother relationship but that did not fix it. The only way I was able to fix it was to use MCC to delete all relationships on one of the sims then redo all relationship in the household through MCC.
