Another update from me after speaking with some people.
It seems like this bug is triggered after using Skillful Sleep and Copypasto. In my experience, it specifically affects same-gender Sim couples (regardless if they're cis or trans, i.e. two gay cis men, two gay trans men, or a cis man and a trans man) who have children together. This bug was not triggered for me in my saves until Skillful Sleep was used.
