This issue has been happening since I purchased the game and seems to come and go. Lately, it's been every time I play and completely game-breaking, making me want to throw my console into traffic.

Interesting! I’ve recently tried using mouse & keyboard on The Sims 4 for PlayStation and the bug never happened.
You might be onto something… maybe it has to do with the code for supporting controllers?Which makes sense this only happens when the cursor is locked to a menu. In free cursor mode it's fine when the issue is happening it's only when it's locked to a menu. This will just randomly happen after a couple of hours playing. Sometimes you can fix it by turning off controller or plugging in a Mouse use it for a second then unplug mouse.
