By sheer accident I noticed the tattoo layer alters the bug. But only for some sims.
On sim A the game doesnt add a random right leg tattoo after adding one manually.
On sim B I added a layer A & B to add a tattoo on layer B and the game doesnt change that again.
On sim C I added a layer A & B to add a tattoo on layer B and when I switch sims or hit play the game deletes layer B, adds layer C and adds a tattoo thats locked behind tattoo skill the household hasnt unlocked yet.
I figured by leaving the other layers empty it triggers the bug. So I added a new layer B again, added a non-cc tattoo to layer A & B aaaaaand it kept my layer C intact when switching sims. I hit play and it finally keeps the tattoo I want.
