It happened in both situations.
I'm getting the message once when loading the game for my infants that were wrongly not part of the dynasty because of this issue: [RL] Children from unplayed Nobles don't receive title/not added to Dynasty | EA Forums - 13195661
Did you try a premade family on the new save or make your own ?
I wonder if the game gets confused with the premades and their constellations.
Yesterday I've only checked on my families, but today I've tested premades from Royalty&Legacy on vanilla.
In each family I've got notification about joining the dynasty (regardless of sim's age) while travelling/editing in CAS etc.
But sims kept being in the same dynasty which originally belongs to... until I've married two sims from different dynasties.
When I've married Jawara Darong with Tybalt Capp and automatically agreed to join her into Capp's dynasty, after reloading the game have decided to also add her kids to Capp's dynasty. So they're in two dynasties at the same time + Jawara kept shown in Darong dynasty despite the fact I've agreed to "move her".I'm attaching screenshots below:
