All Realm of Magic potions, once bottled, have the same description, "Consuming this potion will reset all of your Spellcaster's Perk Points. Allowing them to be reassigned" (or the string 8D090B19 as the DescriptionHash for the Object Catalog).
This only happens once they're bottled and not anywhere else that I can see. In the cauldron, for example, they have the correct description for each potion.
