Sim Dying Emotional Death Randomly

The title pretty much sums up the problem. I was watching her the whole time and she never got hysterical. Which is required to my understanding for a sim to die from laughter. I have mirrors in the both bathrooms and one is on the wall in the hallway directly behind the fridge. Ever since the first time this happened to one of my sims I watch it very carefully and send them to a mirror to calm down. I was allowing her to finish the meal she was cooking on the stove and was going to have her go to the mirror in the hallway when all of a sudden....you see what happened. And I didn't even think to exit and load my last save. So now I am having to fix this mess that should not have happened at "very playful" in the first place.

Update: I replayed the day almost exactly the same and I do think now that she may have been hysterical, but I definitely did not get a dying of laughter notification and I certainly did not have the Grim there. It was instaneous.

More recently I've been having my Sims dying from Hysteria when absolutely nothing had triggered it. One Sim died twice from Hysteria in Strangerville right after fighting the mother plant, first time he was resurrected from another Sim giving Grim the Death flower, only to die seconds later from Hysteria again, he got resurrected again and this time I sent him home and he was back to confident.

I have just been playing my game like normal and the first time she was only very angry but I had changed her traits and right now before she died she is very happy is this happening to anyone else?
